CLINICAL PATHWAYS
The purpose of implementing a Clinical Pathway Program is to assist health care providers in identifying preferred treatments by providing links among diagnoses, treatments and outcomes, and by describing alternatives available for each patient. Practice guidelines provide a basis for evaluation of care and allocation of resources as well as resulting in favorable outcomes for patients with a particular diagnosis.
Oso Specialty Infusion has been successful for over 16 years in developing Clinical Pathways that meet the operational needs of payers, hospitals and medical groups. We work directly with the physician to establish the criteria for a specific clinical pathway. Our Care Coordination Team supports the implementation and outcomes of the specific clinical pathway. Clinical pathways are used to describe and implement clinical standards for the customers we service. They help to provide quality and efficient patient care.
Through the utilization of our program, the physician orders clinical pathways for specific patients and the clinical pathway documents are part of the patient's permanent record and are integrated into the clinical documentation. A detailed description of a process of patient care management that will facilitate improvement or maintenance of health status or slow the decline in health status in certain chronic clinical conditions is derived as an outcome of pathway adherence.
